Case of the Week 332

This week's case was generously donated by Dr. Susan Novak.

A 27 year old woman from Africa had a history of passing worms in her stool since she was a child. After moving to the US, she passed 4 yellow rectangular objects measuring up to 2.0 cm in length x 0.4 cm in width and 0.1 cm in thickness.

These objects were submitted for histologic exam and showed the following:

(H&E, 100x total magnification)

(H&E, 400x total magnification)

H&E, 1000x total magnification; object measures approximately 35 micrometers in diameter.

The objects are consistent in size and morphology of Toenia sp. proglotids. The smaller size of the ova, their thick and striated shells are all pointing toward the diagnosis of Toenia sp. infection. The proglotids did not show any internal organization to help ascertain the species involved. Usually, a younger proglotids would show case the internal anatomy when stained with either HE or Carmine, and a fully mature proglotid would give us the details of the uteral branches.

Eggs look like Taenia, objects look like tapeworm proglottids. Lee

With the size of the worm, I'm guessing D. Latum - the fish tapeworm.
