Case of the Week 358

An elderly man had a lung nodule resected due to concern for malignancy. Below are sections of the formalin-fixed paraffin embedded tissue. Identification?

Hematoxylin and eosin (H&E), 2x objective

 H&E, 10x objective

A careful search revealed only the following single structure ( H&E, 100x objective)

Gomori methenamine silver (GMS) stain highlighted parts of this nodule

    The hooklet indicates an infection with tape worm, possibly cysticercus cellulosa or a hydatic cyst.
    The thick, non nucleated, laminated cuticula is consistent with the supporting wall of a hydatic cyst since cysticercus cellulosa has a much thinner wall.

    Alveolar echinococcosis. The hooklet, lamination of the cyst wall, and inflammation pattern are consistent with Echinococcus. The location in the lung would favor E. multilocularis.

    The hooklet looks so faint in the 3rd slide... maybe if you close the microscope's diaphragm a little you can have a better resolution of it. Sometimes less light is better to distinguish some parasitic elements.
