This week's case features a fun finding in a concentrated wet prep of stool stained with iodine. The case and photograph are from Blaine Mathison and Madison Sant. The object below measures approximately 35 micrometers in diameter. Identification?
A cyst 35 micrometers in diameter with a number of inclusions which appear to be nuclei with eccentric karyosomes and uneven peripheral chromatin. The number of nuclei is greater than 8 leading me to suspect this is super nucleate cyst of Entamoeba coli.
Given a very large size and large number of nuclei, I believe this is a super cyst of E. coli. I don’t recall having found spherules of Coccidioides in stool though I did find them in respiratory samples or tissues. Florida Fan
Measurements and the presence of >8 nuclei indicate Entamoeba coli. This would be reported, but made clear in the report that it is non-pathogenic and likely a result of the consumption of contaminated food or water. Bacterial/viral causes of any gastrointestinal issues should therefore be ruled out.
I want to say Entamoeba coli or histolytica, but I can't quite distinguish whether it's gram positive or negative?
