Case of the Week 720

This week's case is generously donated by Dr. Nazia Nagi from Maulana Azad Medical College in New Delhi. The patient is a young, HIV-positive, adult male with diarrhea. The following structures were seen in a wet mount and modified Ziehl-Neelsen stained stool preparations. They measure approximately 35 micrometers long. Identification?

  1. Well, one more coccidian to consider for the summer: Cytoisospora belli. This is perhaps much less likely to find among the coccidians. A characteristic Cytoisospora belli shares with Cyclospora is that it auto fluoresce under UV light excitation and this allows a quicker confirmation than the acid fast stain.

  3. Looks like Cytoisospora belli. Most common symptom is diarrhea and it is found most commonly in HIV/AIDS patients.

  5. Hi Dr. Bobbi, could you share the procedures for the acid fast stain? we had one isospora pos spec also, and we only can see from the Aurumine stain but our modified acid fast stain not successful. we initially catched them from our concerntration method. Thank you, Michael
