Answer to Case 228

Answer:  Ova (eggs) of Paragonimus sp.

The identification can be made by the characteristic operculate eggs, moderate size (90-100 microns in length), thin shell, and lack of spines.  In some of the images, you can appreciate small "shoulders" or bumps on either side of the opercular opening, another characteristic feature of Paragonimus ova. 


Paragonimus westermani is the most common species worldwide to infect humans, but other species, such as P. kellicotti can infect humans as well.  It is not possible to tell the different species apart by the eggs alone, and therefore, the best identification is "Paragonimus sp."

Case of the Week 227

The following case was generously donated by Dr. Sandeep T from Bangalore, India.

The patient is a 27 yr old lady with complaints of puffy ankles and face and a loss of appetite. On examination, she was pale and had pedal edema extending up to her knees bilaterally. Laboratory investigations showed anemia with a normal WBC count. The preliminary diagnosis was renal insufficiency, but further questioning revealed intermittent diarrhea and one episode of vomiting a 'worm'.

Therefore, endoscopy was performed which revealed the following red brown parasites attached to the wall of intestine, extending from the duodenum to the colon.

Some of the smaller parasites were extracted and are shown below.  They ranged in size from 2 to 3.5 cm in length by 1.5 cm in width.

Answer to Case 227

Answer:  Fasciolopsis buski, the intestinal fluke. 

The first step in making the diagnosis is recognizing that these objects are flukes - flat 'leaf-like' worms (member of the Platyhelminths) with an oral and ventral sucker as shown below.  The oral sucker isn't fully demonstrated in the photo.


The size of the fluke is also important.  As Lukus points out, the flukes that were obtained at microscopy are on the small side for F. buski (2 to 3.5 cm in length), but mature forms are larger (up to 7.5 cm!).  I believe we can see some larger flukes in the endoscopic video. 

Hans points out that there are also other intestinal flukes such as Metagonimus yokogawai and Heterophyes heterophyes to consider, but that they are much smaller in size and wouldn't be in the differential in this case.  Also, the larger thin-walled, operculated eggs (without a shouldered operculum) fits nicely with F. buski.

Of note, edema is a common presentation in individuals with heavy infection, as seen in this case. Other manifestations include diarrhea, fever, abdominal pain, and intestinal obstruction.

Case of the Week 226

A middle-aged male presented with recurrent urinary tract infections and cystoscopy revealed a large bladder stone (7.0 x 5.2 cm) filling the whole bladder. 

The object was removed by open cystolithotomy and sent to the lab for bladder stone analysis.  8.7 grams of material was removed from the 'stone' and analyzed (see below, image courtesy of Dr. Tom Moyer).  It was found to be composed of 10% uric acid and 90% ammonium urate. 


More surprising was the object discovered in the center of the 'stone' (shown below), which was immediately sent to the parasitology lab.

Answer to Case 226

Answer:  Foreign body

This is quite an unusual case, that as Lukus Roberts mentioned, was published in JCM this past year:

We were first cued in the fact that this wasn't a real worm by the homogenous 'spongy' nature of the worm-like object.  You can appreciate this best on cross-section:


It would be possible that a real worm of this size could be present in the bladder - for example, Dioctophyma renale, the giant kidney worm, or less likely, Ascaris lumbricoides - but a real worm would have discernable internal structures which were clearly lacking in this case. 

The object was very resilient and didn't dissect apart, so we also didn't think it was any type of ureteral cast as mentioned by some of the viewers (although this is also a good thought and should be considered).

To work up the case, we first did touch preps of the object and tried to express eggs from it - all which only revealed a few urothelial cells and some bacteria.  We also took a histologic section which showed amorphous eosinophilic material with no outer cuticle.

Fortuitously, we happen to have a jar full of rubber fishing worms that I had sent to the lab as an April Fool's joke 3 years prior.  They had since been sitting in saline as part of our educational collection.  When putting the patient's specimen and known rubber fishing worms side-by-side (both the saline-soaked specimen and 1 fresh from the package), it was clear that they were the same:

To further characterize the nature of the specimens, we performed infrared spectrum analysis of the known fishing worm and the patient's specimen.  The 2 spectra (shown below; patient on top) were indistinguishable:

Once we had this information, we discussed the case with the referring physician who then questioned the patient.  At this time, the patient admitted to inserting the rubber fishing worm into his urethra approximately 3 years prior, but felt sure that the worm had been removed and was not retained in his urethra or bladder.

Answer to Case 225

Answer:  Sarcoptes scabiei var. hominis eggs and newly hatched larva (note it only has 6 legs, while nymphs and adults have 8 legs). You can nicely see the maturing larvae inside some of the eggs!

A 15-month old U.S. child spent 3 weeks in Nigeria with her parents.  On return, the following were seen in stool and blood specimens.

The blood case: only ring forms found, ring diameter smaller than half of infected red cell, infected cells not enlarged. A few ring have two nuclei, one applique form found. Based on these findings, the organism is consistent in morphology with P. falciparum.

(Notes from Parasite Gal - Babesia spp. would also be in the differential diagnosis, although the ring forms are fairly regular in size and shape and there are no extracellular forms or maltese crosses.  Therefore this appearance is more consistent with Plasmodium falciparum.  This also fits with the travel history.)
The second case: A ring of nuclei distributed on the edge of the cell (actually think of it as a clear spherical glass ball and some body just stick a few globs of play dough, if you rotate this ball, you will see different variations in morphology of the distribution of the play dough globs). The identification is therefore B. hominis.

(Notes from Parasite Gal - I like the "globs of Playdough" analogy!  Also, Hans points out that there is taxonomic confusion about Blastocystis, and that there are actually multiple sub-species/genotypes, or possibly multiple species, and therefore it is more correct to say "Blastocystis species" rather than "Blastocystis hominis").
Finally, although there was some debate on the size measurement, the consensus for the last image is Cryptosporidium species, based on the fact that the oocyt shown is less than 8-10 microns, and closer to 6.
