Giardia lamblia.
The leaf shape is the side view of the organism, while the semicircular or lunate shape depict a frontal view.

Pretty florid giardia -- so let's guess that patient is immunocompromised or is this "usual" for symptomatic? On the paraffin pathology side we'd always been told biopsy was gold standard (probably in the era of simple O&P) but I'm suspicious the immunoassays are more sensitive and stool probably nearly as good as duodenal aspirate.
I guess another question is whether all giardia is the same or whether the stuff going around my dog park is different?
