Case of the Week 308

The following case was generously donated by FloridaFan.

The patient is an 85 year old woman in a skilled nursing facility.  She experienced a rash involving her scalp, and so scrapings were obtained and sent to the laboratory.  Identification?

obviously dermodex, but perhaps I am being silly but in the second image, is that a Sarcoptes in the BLH corner? I also think some of the eggs dont really look like Dermodex eggs and more like Sarcoptes eggs...

Oh yes, adult Sarcoptes is there as well with eggs probably belonging to it. I overlooked the adult, but I was confused by the etiology of rash which is not so common in demodeciasis, is it? I think, Demodex is thought not to cause problems usually.

Demodex over the upper part of the photos, and empty "egg shells" of Sarcptes scabei in the lower half of the first photo. The lower left hand corner of the second photo shows the scabies mite.

Demodex and Sarcoptes by the looks of it. It's likely that Sarcoptes is causing the rash, though in some cases Demodex can cause Sarcoptes like rashes.

The reason why I was a bit hesitant about an identification Sarcoptes is that I think adults would be a lot bigger than the eggs...though this one could perhaps be a nymph?
