Happy Fourth of July to all of my readers in the U.S.! This week's case is a good reminder to think about food-borne illnesses as you go to picnics and cook-outs on this weekend.
The patient is an adult male with "explosive" watery diarrhea (like fireworks - sorry, bad pun) for the past several days. Round, slightly wrinkled-appearing objects measuring approximately 8-10 micrometers in diameter were seen as negative outlines on routine Trichrome stain:
These objects also stained clear to bright pink-red on modified trichrome stain (if you use your imagination, you can say that this slide is red, white and blue):

The size, non staining with Trichrome stain oocysts taking a "ground glass" appearance and then stain acid fast, all clues are consistent with Cyclospora oocysts.
Folks, we started to see Cyclospora oocysts here in Florida last week, together with a little increase in the finding of Cryptosporidium. A little early to say if it was sporadic finding or the beginning of an outbreak.

That's a good observation Florida fan. We've had 10 cases at Cyclospora detected in the past month, which is more than we detected in all of last year. Furthermore, there is no pattern to the states from which the cases were detected. They've come from all across the Midwestern, North Eastern, and southern states. Therefore, I think there is a small outbreak going on.
