Case of the Week 512

The week's case is generously donated by Dr. Peter Gilligan, Cody Craig, and Leslie Dodd. The following objects were noted on a Papanicolaou-stained rectal smear from a male patient. They measure approximately 5 to 10 micrometers in diameter. Images were taken using a 40x objective.

I have no experience with PAP-smears, but looks like Iodamoeba butschlii to me...

I would go with Iodamoeba butschlii too but the second image with the wavy exocyst reminds me of Acanthamoeba (though the source makes this unlikely)

The “wavy exocyst” in the picture could also be the ectoplasma of an amoebic trophozoite...no...?

Folks, it still Monday yet. Can't believe there are so many respondents already. First the size of five to ten microns is compatible with that of I. butchlii, the large vacuole though not filled with iodine stained glycogen (it's a Pap smear) makes this amoeba more than likely being I. butchlii. The site is rather unusual for Acanthamoeba. The organism is too big and also picks up the stain making it not a Cryptosporidium oocyst. It does not have any flagella nor axiostyle to be Pentatrichomonas. If it turns out to be an Acanthamoeba, then Dr. Pete certainly threw us into a hoop.

I was consulted on this case, so I'll have to keep my mouth shut this time LOL. But I am enjoying the answers so far!

Initially I lept to cyclospora vs cryptosporidium. However, the size range is too wide for either of those and there are very clearly defined nuclei.
On second thought, and final answer, I think they are iodamoeba buetschlii cysts.

Size of cyst, presence of large vacuole, I believe the species is Iodamoeba butschlii.


Is Pap stain commonly used on fecal smears? I confess my ignorance. Veterinary parasitology seldom uses stains and fecal smears.

What also pushes me towards Iodamoeba butschlii (apart from the vacuole) is the large karyosome that lacks a ring of peripheral chromatine.
