Case of the Week 685

 This is week's case is from our long-time contributor, Idzi Potters from the Institute of Tropical Medicine, Antwerp. The following structures were seen in a iodine wet preparation of concentrated stool. Identification?

? negatively birefringent - look like urate crystals

Charcot-Leyden crystals (residue of eosinophils). Occasionally seen in stool of patients with hookworms or amebiasis.

The needle like objects appear crystalline. Most likely urate crystals.

I also think that it is monocot crystals, such as pineapple, asparagus, etc. To differentiate them from fatty acid crystals, we can heat the slide: if they are monocot they remain, if they are fatty acid, fat droplets would form.

Agree. Fruits or vegetables Crystals can be confused with Charcot-Leyden crystals.

Charcot Leyden crystals. A breakdown product of Eosinophils. So this could be an indication of a potential helminth infection, or their presence could be due to another inflammatory reason (e.g allergy)

Fruit crystals. Very slender and long. Not looking like the Charcot Leyden ones.

After further search, I believe these are fruit crystals as illustrated in medical-labs.net. More than likely pineapple crystals.
