Case of the Week 647

 The following objects were seen on Giemsa-stained touch preps of a skin biopsy from a patient with a slowly enlarging ulcer. He had recently travelled to Costa Rica. Identification? What additional testing is indicated?

Amastigote of Leishmania sp.

According to Leishmania sp. amastigotes

Identification of the isolated species with monoclonal antibodies, isoenzyme analysis, hybridization with DNA probes or polymerase chain reaction (PCR). It is especially important in travelers to Central and South America to rule out infection by L. braziliensis.

Wow! We already have three identifications for Leishmaniasis. This shows the enthusiasm of loyal readers. Despite the advances of science, the expert eyes on the microscope is still a major component of clinical diagnosis.

A case of cutaneous leishmaniasis (L. braziliensis or L. mexicana), indeed. We can see clearly the nucleus and kinetoplast from the amastigotes and the host cells nucleus. PCR is recommended.
