Case of the Week 671

This week's case is by Idzi Potters and the Institute of Tropical Medicine Antwerp. As always, he has an interesting challenge for us! the following were seen in a wet preparation of the stool concentrate. Identification?

Although the last picture does show some mamelations on the shell, knowing that it comes from Idzi I would hesitate to take it at its face value. I wonder if the first picture shows an inconspicuous operculum. Very interesting case indeed Idzi, bravo.

Agree with unfertilized eggs of Ascaris lumbricoides. However, although unlikely, it appears to be a mixed infection with Trichostrongylus spp.

Great thoughts everyone! For those of you familiar with Idzi's cases, you were probably expecting something more complicated. Indeed, he had suggested that I show only the decorticated eggs, and save the clear-cut Ascaris eggs for the answer. Therefore, you can blame me for making this too easy and having you look for the complexity that wasn't actually present!

Wonderful bluff Dr. Pritt, we need all aspects of this widely distributed parasite. In fact it was the first egg I saw in my student days, now it still can give a challenge like in this case.
