This week's case was generously donated by Dr. Mike Mitchell and Laura Derderian. The following structure was seen in a stool concentrated wet mount specimen. It measures 27.5 micrometers in length. This was the only structure present in this specimen. Identification? What additional testing might also be useful in this case?

Sure looks like an egg of Cystoisospora (formerly Isospora) An acids fast stain or PCR could confirm this but is not really necessary

Beautiful oocyst of Cytoisospora belli. They auto fluoresce brightly under UV light excitation and will stain acid fast too. Our team members prefer the auto fluorescent methodology as it is simple and less time consuming.

Yes I was about to say Isospora. I’m old-school and haven’t done Parasit. in years!
Old name was Isospora sp. and oocyst. Do a modified acid fast stain. Awesome find, too. Autofluorescence blue with right filter setup.
