Case of the Week 782

This week's case was generously donated by Dr. Adrienne Showler. The following image is from a video capsule endoscopy performed on an immunocompromised patient with diarrhea. The patient has not travelled outside of the US and has no other risk factors for parasitic infection. Numerous of these objects were seen - none were moving. Identification?

Looks like an insect larva (Brychius or Limnius larvae) / digested larvae of some myiasis, but the number of segments looks over 12-13 (unlikely to be an insect) and the size is unknown.

I’m confused by the brown pigment. It does appear to be a nematode in shape. There is no measurement given and this makes it difficult to reliably identify. Could it be a full blown case of Strongyloides stercoralis auto infection?

This is a total enigma. First the brown coloration of the object does make it likely to be a parasite. Second none of the objects were moving, parasites do move more or less but when alive they do. I believe this to be an artifact. As far as parts of a banana? Who knows, this could very much be like in case 137.
The entire mystery makes my mind going bananas and reminds me of the verse “objets inanimés avez-vous donc une âme »

These make me think of mosquito larvae passing through. Or something of the likes. Not a parasite for sure!
I’d like to know what kind of weird stuff this patient has been eating…
