Answer to Case 781

 Answer to Parasite Case of the Week 781: Demodex sp.  The legs and gnathosoma (with mouthparts) are nicely demonstrated:

I can't quite tell if this is D. brevis, or Demodex folliculorum with a scrunched-up tail. 

We've had some great cases of Demodex on the blog over the years. Here is the list in no particular order:

Case 58 - the very first Demodex that I ever posted - 16 years ago!
Case 190 - a beautiful example of D. folliculorum in a hair follicle in an H&E-stained histopathology preparation. It was donated by Dr. Holkmann Olsen and is the best example I've ever seen of the mouthparts and legs in a histopathological section. 
Case 308 - and finally, an amazing case by Florida Fan that shows Sarcoptes scabei and Demodex folliculorum mites in the same image. 

I've been blessed to have so many fabulous case donations over the years. Many thanks to Dr. Rasool Jafari for donating this fascinating case.

Answer to Case 503

Answer: Demodex folliculorum, the follicle mite.

What is it doing on the toe? Well, toes have follicles too! It's true that we generally think of these little mites on the face, but they can be found anywhere on the skin. Interestingly, dogs (and possibly other animals) can get severe infection of their paws from Demodex species (called demodectic mange). In humans, Demodex are mostly considered to be harmless inhabitants of our skin.

Here is a nice side-by-side view of the 2 images in this case:
Note the presence of 8 legs in the adult form (mites are arachnids and not insects). Thanks again to Florida Fan for these lovely images!

Answer to Case 601

 Answer: Sarcoptes scabei, the 'human itch mite'

As noted by Kosta, Patrik, and Anon, this is a larval mite, as evidenced by the presence of only 6 legs. Mites are arachnids and have 8 legs in their nymphal and adult stages. 

As noted by Old One in a previous post, Sarcoptes are round to ovoid when viewed from the back; when viewed from the side they are ventrally flattened and dorsally rounded (similar to a turtle). They possess short stumpy legs, and have no internal or external respiration apparatus (stigmata or tracheae). The ventral surface contains a number of chitinized plates called apodemes, the dorsal surface is partially covered by wide-angled, V-shaped-spines (>). The cuticular surface is sculptured into numerous parallel ridges which superficially resemble human finger prints, and the anus is at the posterior end of the mite (this is the characteristic used to differentiate Sarcoptes from Notoedres which has a dorsal anus and sometimes infests humans) It's dorsally-located anus makes it appear subterminal. Check out his additional comments in the comments on this case regarding Notoedres cati. This cat parasite can cause transient pruritic lesions in humans that closely resemble scabies.

Case of the Week 503

This week's case is a fun identification from Florida Fan. A podiatrist submitted a toe nail sample from an elderly female for fungal smear and culture. Fungus wasn't seen using KOH Calcofluor staining, but instead something else was noted:


Identification? And any thought on what it's doing on a toe?
